Here are my suggestions for what you're looking for:

• Change the trigger for each question slide: instead of jumping to the next slide, submit the interaction when the user clicks the button.

• Create Button sets - select both radio buttons (true and false) for each line, right-click, choose Button Set, then New Set. This will make sure the user can only select one of each.
