Revisit slide triggers not working as expected

It is really hard to tell, without seeing the actual. project, but I am going to take a wild guess. I'm not sure what other state AnswerButton might be in, but if it is a built-in state, the top trigger won't work. All the built-in states are like the rest of us; they consider themselves to be perfectly normal. So if the button is in "Visited", for example, it will report itself as Normal, and this trigger won't fire.
