I have never had any issues like what you have described. A couple of thoughts ... Does it happen when you publish to the Review site or when you publish locally? Could it be the quality of the MP4 file? If it is too high or too low, it may cause issues. Also, there is a setting on the Video Tools tab when a video is selected for video compression. Have you tried adjusting that? Those are settings that I would look at to see if any changes make a difference. Good luck!
