Diagnosing video playback issues

Can anyone provide diagnostic insight into this scenario? Given:
(a) Recorded (mp4) training sessions inserted into slides as video, with native video controls shown. Controls are "covered"  until video is completed, when then the media completed variable (IE Section1Complete)  indicates completion  (TRUE) and "uncovers" video controls through object states.
(b) When revisiting: set to "Reset", where trigger ensures control is still "uncovered" and users can FF/RW as needed.
(c) videos no longer than 10:00 in duration (most smaller).
(d) Video placed on timeline to start at :01 to allow Base Layer to splash,  video automatically starts
Symptom: Video randomly inaccessible, no longer displayed.. originally thought to be related to a "pause" but has happened while being watched.. like it no longer exists on the timeline.
