Multiple Fill In The Blank

Hi All, 

I am working on developing a question with multiple fill in the blanks using conditions to select correct/incorrect based on text variable responses. When I test / fill in the variables, I get a message saying I must complete the question before submitting. I am not sure what I am doing wrong. For your review: Test Question - I have also uploaded the file. 

Thanks to Michael, I was able to fix this problem by making sure the trigger for submit was below/after my change state of triggers w/conditions. Still looking for input on the second part though :) 

Also, it would be ideal for each entry to be scored individually rather than as a whole, if anyone knows a work around for that, I'd really appreciate it!
