SCORM Course not passing completion

We have a SCORM course that does not pass completions to SCORM Cloud for a few users. The completion is recorded successfully for few users; however, for few users the status is passed as "Incomplete" even though they have completed the end of course Quiz successfully and clicked on the "Exit" button within the course. 

I am attaching screen-shots of the "Reporting", "Tracking" and "Trigger Wizard" for this course and the debug logs. 

Request your assistance, as we are unsure why its works for some users and not for others.
