I'm looking to create a survey with multiple Text Entry Fields per page. Is there a way to compile and receive input answers?

- Storyline submits the answers from Survey Question slides to the LMS if you include a Survey Results slide in the course.
A Short Answer survey question slide only reports its 1 text-entry field.
I'm sure JavaScript could report multiple text entries from a single slide, but that's not my area of expertise.
