Embedding a form into Rise
I would like a very simple form to be embedded into Rise to collect the name and email of the person at the end of the course. Google forms look bad as it adds scrolling within scrolling. Any other suggestions? I want to host on the web and not in our LMS because our LMS requires way too many steps for guest/outside users to get into.

Hello Alyssa, thanks for this helpful answer which I only need now. I want to embed a form in Rise but not necessarily to collect data but rather for the user to answer some questions related to content that he/she just watched and then download the form as pdf to use it outside the course. Is that possible?

Hi there, Eric! Have you tried an embed block? I created a quick survey using SurveyMonkey to add to my Rise course. I'm just collecting two fields of information, so maybe you'll like the way this looks better than Google Forms.

I gave up on Rise. The limits to what can be embedded was a small issue. Not meeting accessibility requirements, though, made it unusable for my context.

Thanks for this post everyone. I am trying to follow Tom's Blog email that I received on 11/7 re. doing this exact thing using Jotform or Google Forms. I cannot get the Jotform to embed into my RISE course. When I embed it using the code given by Jotform, it displays like this in Preview mode (see image).
Any ideas?

Trying to embed a Microsoft Form into a Rise course and tried all of the instructions above and am still getting an error message. Suggestions?
Hi there, Natalie. Check out this sample I built by embedding the Microsoft Form using a multimedia embed block in Rise 360. I listed the instructions there, too.
Here's some helpful troubleshooting information when working with embed codes.

your sample is not working - seems like code was changed at either Rise og Microsoft. Is there another way to get i working again?

correction - the sample is not working in firefox. Just found out its working in chrome.
will it be working in firefox?
