What's the "When revisiting" setting in the Slide Properties? If it's "Reset to Initial State," the buttons get deselected, which could make the question incorrect.
From what I've seen, it's best to use "Automatically decide" for question slides. That lets Storyline use its built-in functions for when the user (re)takes or reviews a quiz.
I agree with Aaron. It does not look like a choice was made. Can you share your file?
