Mystery object in "Invalid Answer" layer's focus order

Hello- I am finding a tiny "mystery object" in the focus order of the built-in Invalid Answer layer. NVDA reads it as "Clickable." I can tab to it, but nothing happens when I select "Enter." However, I can select it with the mouse, then it goes away. Please see screen shot. 

I have explored the Player's text labels - is it the Notification: Invalid Feedback Title? I cannot delete this text label, and I do not know how to access the actual Invalid Answer layer to investigate the focus order.
