Navigation madness

I'm trying to create a menu system, but I'm experiencing erratic behavior. Please help me if you can.  The first issue is shown below. When I click on "Assessments," The word becomes underlined. I need to click a second time to actually go to that section.

Underlining again appears for the next Maintenance, however, the link goes to a non-existing screen.
