Do I have to redesign my slides from the Survey layout I selected within Storyline 360 to a custom layout to achieve this?
Yes, you'll need to create new slide(s) for your custom Likert statements. Just insert a standard slide (not a Survey question), and add the text, buttons, and other objects you need.
