Is it possible to build a survey or questionnaire in Articulate 360? One that you wish to record the results?

Yes. There are survey questions included in both Storyline 360 and Quizmaker 360. The instructions are below:

Quizmaker 360: Adding Form-Based Questions
You can track the survey with a results slide and send this to your LMS. Each individual LMS can handle surveys differently, so I'd advise testing this.
I have one more question about Quizmaker and Storyline 360 quizzes.
Is there a way to print an online quiz/survey (for those who do not have online access)?
