Survey tool Storyline360

I am a beginner to Storyline 360.
I am designing a Survey type tool used by Assessors to Observe operator competency when using a machine. The operator is then assessed as either Competent or Not Competent on each activity. The Operator is observed through 4 stages: SETUP: STARTUP: SHUTDOWN: CLEAN. This observation may happen on different days and shifts. We then need a result to be tabulated across these 4 stages telling us if the Operator result of Competent or Not Competent. My questions are:
- Is it better to design each stage as a separate survey as the observation may happen at different times / shifts?
- Is the Survey the right option, or is a quiz better?

I'm not entirely persuaded that Storyline is the best tool for what you are describing, though it may be workable.
As I understand, what you are doing is asking Assessors/Evaluators to "observe Operator competency" and then rate/evaluate/assess the Operators.
While you could certainly use Storyline to evaluate someone's task performance, you would have a very difficult time configuring a story module to actually capture someone's performance on the four task stages you are describing (Setup/Startup/Shutdown/Clean). The only way I would think you could do so would be to videotape the Operator; upload the video to Storyline and publish to your LMS; then have the Assessor review the video and mark the person as "Competent"/"Not Competent."
Doing this type of "independent evaluation" would be possible if you have one or two Operators. But for 12 to 20 Operators, this would be an ongoing nightmare.
You are better off creating a calibrated checklist of behaviors that the Operators need to demonstrate at each task stage, and then giving the checklist to the Assessors. Tabulate the results and you will have your outcomes (competent/not competent).

Thanks for that JHauglie. Much appreciated. We are currently doing it through hard copy checklists, I was looking for a digital solution using iPads or Laptops. Apologies on the late acknowledgement.
