Do you want these notes to be individual private reflection that the learners can save on their machine/print physically or data that would be recorded in an LMS or LRS?
The simplest solution for the former would be to use the built in "Print Slide" Trigger, which learners can then choose to print the storyline slide (layout and input field and all) as either a physical page or as a PDF from their printer dialogue screen.

Unfortunately I'm not at a place with my understanding of xAPI (or other potential solutions) where I can recommend solutions for you if you're looking for the second of these two, but either way I wish you the best of luck!
