Triggers going to incorrect slides - I really need help asap with this one if possible

Hi again,  Storyline has crashed again and I have another situation with a mysterious trigger.  It is not on any of the master slides including the feedback master.  I have removed the information images etc from this module and am attaching it to this post.  

The issue happens on slide 1.5 - survey question (accessed via the Marked complete button on slide 1.2 Instructions (Cont) .  The submit button indicates jump to slide 1.1 Instructions but it actually displays slides 1.6 Third time.  

The story view shows two triggers from slide 1.5:

1. to slide1.1 which is correct and the trigger can be selected.
2. to slide 1.6 which is incorrect and the trigger cannot be selected.

There are no triggers on the feedback master for the survey slide.

Any suggestions would be appreciated.  I also need to know why this is happening, and if it relates to Storyline shutting down, then why that is happening too.
