Verbiage for RCA Introduction
I am working on a CBT for a Root Cause Analysis that was completed on an error that occurred in May. I am providing training on the initial procedure as a reminder, so the issue doesn’t occur again. The audience is aware of the issue that occurred, and what they did wrong causing it to happen. I’m looking for some verbiage to introduce the CBT without making anyone feel bad about it (don’t want anyone to think they’re being punished or called out). Any suggestions?
