Seeking advice: ID Consulting with stakeholders +help with design intake form

Hi everyone,I am looking for some expert guidance on conducting effective instructional design consulting meetings with stakeholders. When meeting with a stakeholder for the first time regarding a new learning project, what key points should we be covering?Specifically, I would love insights on:

• What essential topics should be discussed during the initial meeting?
• How to structure the conversation to uncover goals, expectations, and constraints?
• Any tips or best practices for gathering accurate content and learner context?
• How to align on timelines, delivery formats, and success metrics?

Additionally, I am in the process of creating a Design Intake Form and would appreciate any examples, templates, or suggestions on what to include in it to ensure all critical information is captured right from the start.
