Course Duplication Issue - Original Content was Wiped and Replaced

Hello,

A Course Collaborator (Manager) attempted to duplicate a course which I had created but the content was replaced in the "duplicated course".

Instead of keeping my content it kept the titles, description and lesson names but wiped the content and added a few lessons which were not part of my course.

What could have caused this and how can we prevent it from happening again?
