I would like a very simple form to be embedded into Rise to collect the name and email of the person at the end of the course. Google forms look bad as it adds scrolling within scrolling. Any other suggestions? I want to host on the web and not in our LMS because our LMS requires way too many steps for guest/outside users to get into.
I gave up on Rise. The limits to what can be embedded was a small issue. Not meeting accessibility requirements, though, made it unusable for my context.
Hi there, Eric! Have you tried an embed block? I created a quick survey using SurveyMonkey to add to my Rise course. I'm just collecting two fields of information, so maybe you'll like the way this looks better than Google Forms.
Thanks for this post everyone. I am trying to follow Tom's Blog email that I received on 11/7 re. doing this exact thing using Jotform or Google Forms. I cannot get the Jotform to embed into my RISE course. When I embed it using the code given by Jotform, it displays like this in Preview mode (see image).
I'm running into an issue when embedding a Google form into Rise and in editing and previewing I get a reCAPTCHA test (see screenshot). I'm using iframe to embed the form. Is there something I'm missing?
I'm sorry you're having some trouble with this! Our team did some testing and couldn't replicate your issue. Let's get your course over to our support team. You can share your course with them here.
Can you share your link so I can try it out on my end to see if it's working? If you cant share it publicly, please get in touch with our Support Team so they can take a look. You can submit a case here.
In the meantime, if there's anything else I can do to help, please let me know!
Hi there, Natalie. Check out this sample I built by embedding the Microsoft Form using a multimedia embed block in Rise 360. I listed the instructions there, too.
Hello Alyssa, thanks for this helpful answer which I only need now. I want to embed a form in Rise but not necessarily to collect data but rather for the user to answer some questions related to content that he/she just watched and then download the form as pdf to use it outside the course. Is that possible?
Hi Crystal, I have managed to get this working but noticed that it jumps straight down to the embed block when opened. This means students have to scroll back up to the top of the lesson which is problematic. Any ideas why it does this?

Hi Anne, I posted the exact same question earlier today and no hits. Have you gotten anywhere?
Real shame to see no replies or answers here as I am also really interested in how to do this for Storyline and Rise.

Hey Nat! Happy to help. Here's a quick sample Rise blocks lesson with an embedded JotForm. Check out this video to see how I put that together!
Thanks so much Alyssa! I get it now.

Here's some helpful troubleshooting information when working with embed codes.
Let me know if you need more help!
your sample is not working - seems like code was changed at either Rise og Microsoft. Is there another way to get i working again?
correction - the sample is not working in firefox. Just found out its working in chrome.
will it be working in firefox?
Hi there, André. The sample worked for me in Firefox version 69.0.3 for Mac and Windows. What version of Firefox are you using?
hi using firefox version 70.0.1 - found out i need to turn off 'enhanced tracking protection' - then it works :)
Thanks for the update, André. I'm glad you got it working!

Hello, Heike!
You could create a fillable form using a third-party tool (like Google Forms). The form can be embedded in your lesson using a multimedia embed block.
Google Forms has a setting that will automatically email responses to the person filling out the form. Would that be an option for you?
Thank you, this worked perfectly.

You can add a Continue block before your form that will hide it until needed.
