Storyline360 Printable Multi-Question Free-Response Reflection Template

Turn learner reflection into something they can take with them. This customizable Storyline 360 template includes five required free-response fields and a Save My Commitment button that creates a clean, print-friendly summary of the learner’s responses. Required-field validation prevents learners from continuing until all five prompts are complete, and the printable version preserves longer, multi-line responses. Learners can print their completed action plan or use their browser’s Save as PDF option to keep a digital copy.

The interaction uses Storyline text-entry variables, trigger conditions, and JavaScript to create the printable output. Both the on-slide experience and print layout can be customized, including the prompts, colors, fonts, title, and footer. The included JavaScript is commented to make key customization points easier to find.

Good for: reflection activities, action plans, learning commitments, goal setting, post-training application, and other activities where learners benefit from leaving with a copy of what they entered.

  • Great ideas! 😊 I hadn't thought about the possibility of emailing the responses directly to instructors, but that could be even more useful than generating a PDF in an educational setting. I also like the idea of embedding it in Rise 360 as a personalized takeaway. Thanks for sharing your thoughts!

  • Thanks! 😊 I really appreciate you sharing it with the community. The template is very useful and has already given me some ideas. I work at a university, and I'm exploring the possibility of using it as an open-ended questionnaire where students submit detailed responses to a practical or clinical case for instructors to review and assess.

Best of luck to you too, and thanks again for your help!

Nice! Yeah - I was able to use AI to help me re-write the JavaScript as needed in order for the printout to be branded for our company. I wonder if you'd be able to have it trigger an option instead of it printing to PDF to have it share as an email to the instructors. I also planned on having this particular slide be a Rise360 block embedded in the flow of learning so they have a personalized takeaway.

  • Hi, and thank you for your quick response!

I was able to get it working. It turned out to be a browser-related issue on my end. After trying with a different browser, both the Storyline file and the Review 360 publication worked correctly.

Thank you for offering to help troubleshoot and for sharing the template. It looks great, and I’m looking forward to exploring it further.

    Thanks again!

You're very welcome! So glad it worked out and that I was able to provide something helpful. I've used a lot of templates I was able to get from this community as I've learned the tool over the last two years, and I decided to finally give back now that I feel at least somewhat able 😉 Lots to learn all the time! Best of luck to you.

  • Hi! Thanks for sharing this template.

I'm having trouble publishing the provided Storyline file to Review 360. The project publishes successfully, but when I open the Review 360 link, the course appears completely blank (white screen) instead of displaying the content.

Has anyone else experienced this issue with this file? Is there a specific Storyline version required, or is there anything that needs to be updated before publishing?

Any suggestions would be greatly appreciated. Thanks!

Hello anacatena-ce058 - I developed this and would be happy to try and troubleshoot any issues you're having. I just created this post last night and I believe you're the first to try it out! From what I'm seeing both the Storyline360 download and the Review360 links are working. Are you trying to download the Storyline360 template so you can make it your own, or you're trying to use it as is? Can you provide any screenshots of the process you're taking so I can try and help you identify what's happening?
