Thank you, Articulate, for including this demo in your E-Learning Examples section! There is a blog post that lists the authoring tools at: https://tracycarroll.net/using-pre-test-activities-to-tailor-content-in-e-learning-206/ I'm not sure why the sentence "*This template will work for folks using Storyline 360 (included in Articulate 360), Storyline 3, or Storyline 2" is included in the description of the example, though. That seems to imply that the template is downloadable, which it is not. Also, the example was created in Storyline 360, which means it would not compatible with Storyline 2 at all.
