I’d first confirm that the Focus Order itself is correct before changing the slide.
If NVDA is still skipping the first object, I’d check the previous slide, especially if it contains long text and custom navigation buttons. This is a documented Storyline accessibility issue, so it may not mean that the Focus Order was configured incorrectly.
I’d also test the same course using the default Player Navigation Buttons where possible, and test with another screen reader if available.
The important part is to test the published course, not just the Focus Order panel in Storyline.
