As Ron shared, a helpful first step would be to review any Triggers that have been added to the Results Slide from your course. Would you be comfortable sharing a copy of the .story file for testing on our end? Feel free to upload it here in the discussion, or privately through a support case.
Just to confirm, is the platform being used to host the course, an LMS? If so, and the LMS is not reporting pass/fail status accurately, try testing how it will behave in another LMS like SCORM Cloud to isolate the behavior. A problem with a course can be observed regardless of which LMS you use, however, LMS-based issues cannot be replicated in a separate LMS. If you don't encounter any issues in SCORM Cloud, please work with the support team of your LMS so they can address any reporting issues you may be experiencing in their environment.
