Quiz not allowing learner to progress

I have a .story with a 10-question quiz at the end. In testing, no problems. 

We had one learner, however, who got stuck on the first quiz question (1/10 questions).  It looked like the screen shot attached. It does not let her rechoose an answer for that screen and continue. The LMS shows her course as Incomplete with a 10%. 

When an LMS admin impersonates her, she gets the same with multiple browsers. (The user was using Chrome.) Those same browsers, when the LMS admin logs in with a different testing account, have no issues  moving through the quiz.

Why would it not let her past this? What happens is she can select the small X on the bottom red bar to exit, and that bar on the bottom goes away, but then none of the radial buttons will select to allow her to choose another answer.  I have not seen this screen before (the one in the screen shot.)

The .story is huge so I just deleted all but the last scene with the quiz, and attached that.

    This is certainly an odd situation. I tested the .Story file you attached without hitting an error. I understand the LMS admin impersonated the user and had the same issue. Did they use the same computer or a different computer? If it's the same computer, I suggest they try clearing their browser cache, and ensuring this list of network endpoints are enabled.

    Since this only happens for one user, it leads me to believe the issue isn't with the course, but the environment in which the course is viewed, or with the LMS. A good place to start is to test the course in SCORM Cloud. If the course works as expected, I recommend reaching out to the LMS administrator for assistance. If the course doesn't work, please let me know here or privately in a support case so we can continue troubleshooting!

      It is not on the same computer. It was her impersonating the learner on a completely different computer that it was reproduced-- and in multiple browsers. 

      For that screen shot I shared-- where did if come from? That red bar on the bottom is not a part of any of my project slides or templates.
