Review Quiz Incorrect/Correct Answers Not Working As Expected

I'm happy to share that we just released another update for Storyline 360. In Build 3.104.35448.0, we’ve included essential fixes and new features. One of the bugs we’ve fixed is:
- When in Review Mode, the automatically generated check marks were observed to not meet the requirements for WCAG 1.1.1
Launch the Articulate 360 desktop app on your computer to take advantage of this update, and click the Update button next to Storyline 360. You’ll find our step-by-step instructions here.

I'm sorry you've hit this snag regarding the Review Quiz settings; I'm happy to help troubleshoot this. I started by creating the attached Storyline project with five questions and a Result slide. I set both Review Quiz buttons in the Success and Failure layers to Review only incorrect questions.
Tests show that the Review Quiz functionality works as intended; it only shows the incorrect questions when the learner passes or fails. Feel free to download and test my sample project.
If you see a different result with your course, we're happy to help troubleshoot your Storyline file if you're comfortable sharing it here. You may also send it along privately in a support case so our talented support engineers can look at it. We'll delete it when we're done testing!
Thanks Eric for sharing the sample file. Yes, the sample file is working as expected.
But, I am experiencing an issue where both correct and incorrect questions are shown while reviewing the quiz in my story file.
When I compared it to the sample file, I noticed that the difference is Question bank. I used a question bank (for the purpose of randomization and pooling functionality) to display the questions whereas sample file didn't have a question bank and the questions were displayed as normal slides.
In the sample file, i created question bank with the existing questions and used it to show the questions. I faced same issue with the Question bank format. It also showed both correct and incorrect questions while reviewing/retrying the quiz.
Review quiz: I noticed that the correct answer questions were shown if they came after the incorect questions.
