Survey tool in Rice
I found a great example of someone integrating a survey into a Rise course. Learners could select a number from 1 to 10, answer yes/no questions, and indicate their opinion using a slider. Only one question was shown at a time, and learners had to click the button in the left corner to proceed to the next question. It seems like the answers are stored somewhere so that they can be evaluated.
Does anyone know how this was created?

It doesn't look like that survey was created in Rise or Storyline. Do you know where it originated from? If so, maybe others from the community can provide tips on integrating that type of survey in Rise 360.
In the meantime, I'm happy to share that we're tracking a feature request to introduce a Survey Question Block to Rise 360. I've included your voice in the report!
