Can't get RETRY to work correctly

I have a form that the student has to fill out, there is if/then language to tell if ALL fields are correctly completed.  There is also a reference document that shows on a cue point.

When the student misses one of the fields they get the RETRY option, unfornately when they get back to the form fill screen (sometimes the reference document doesn't show so I added a button to open it back up as needed) the Submit button doesn't show.  

I tried setting to  Reset to initial state, and it makes the submit button visible but loses count on the attempt number (they only get 3 tries).  If I set to Resume Saved State the counter works but the Submit button doesn't activate.

Any help would be greatly appreciated.  I have attached the slide here for your reference.
