I appreciate you chiming in and sharing what you are experiencing with your course when uploaded to your LMS. You mention that this is not happening during preview but specifically in your LMS.

Are you able to replicate in another LMS, such as SCORM Cloud? If so, please share your .story project file with our support engineers here. It will be deleted when troubleshooting is complete.
