Hotspot selection is removing other layers from the slide

Hi all,

I am hoping someone can help with the attached file.

The learner is required to click on all the hazards and then select submit. When a hazard is correctly chosen, a green tick (a new layer) will appear on the hazard. It all seems to work fine apart from the puddle layer.

For example, the learner can click on the "wire", the "broken chair" and the "rug", and the green ticks will appear as necessary. However, if the learner then selects the puddle, all the other green ticks disappear and seems to reset.  The learner then has to reclick on all the hazards.

I cannot see why this is happening as the "puddle" layer is set up in the same way as all the others. Please help!
