Integration self assessment - Rise 360

Hello, 

I'd like to integrate a self-assessment into Storyline.

In this self-assessment, I'd like to be able to freely assign points to each of the proposed answers, and then calculate scores and give a personalized interpretation of the results based on the choices selected.

I don't know how to proceed. Could someone help me? To understand the logic behind the self-diagnostic, I've attached it to my message.
