Interactive form with specific outputs?
You could use variables to track a user's responses, and then use triggers with conditions to tailor the feedback.

Could I make an interactive form or will it appear as slides?

Using Storyline means working with slides. The more responses you want to collect, the more slides you'll need. That's why variables are needed: a variable's value can be referenced and used in triggers anywhere in a course.
How the slides look is completely up to you.

This is helpful, thank you. Wouldn't it be lovely if there was a template ready and all I had to do was fill in my content? Alas, it sounds like it's going to be much more complex.

We have some 50 questions so I would like it to be much more concise, like a form rather than a large slide document that I'm giving the user.

You can use Storyline in conjunction with a fillable PDF. The process is a bit involved but there are numerous discussions on this topic here in the forum. You gather the variable values in Storyline and pass them to a PDF form you create with the same variable names. The result is that the user has a printable form populated with the variable values you gather and summarize in the Storyline project.

This is very helpful, thank you. It seems, though, that using Storyline is unnecessarily complex. I have access to some survey tools and our organization has just started using LimeSurvey, so I will start there and see if I can use that platform for something. I don't have the expertise or skill set to do this level of back-end sophistication.
