Multiple response quiz question using free form/pick many
I could use some assistance, please. This is a multiple-response quiz question. I have 9 possible answers with only 5 correct responses. I used the Free-form question tool, using “Pick Many” as my template. I thought I set up everything correctly, but when I test the question, I get this response:
I am not sure what is not complete.
The correct responses are in this screen capture.
What am I doing wrong? What am I missing? I’ve attached my file for your review. I appreciate anyone’s feedback or recommendations.

No need for assistance or guidance. This morning, having spent time away from the project, I was able to resolve the problem. I had not grouped the input object, i.e. the check mark, with its corresponding textbox containing the description. Once I did that, everything worked correctly. Sorry to intrude in everyone's busy schedule.
