Publsihing survey results

Hi, I am building a digital course in our Workday Learning (LMS). There two lessons to the course: 1. An online learning module in Rise with a built-in SL quiz. 2. I have created a 'competency' check where users are given scenarios and they have to type out there answers. 

I need the LMS to show me those answers. I have built a test file and when I publish to the LMS (I have a survey results slide - I learned that from a previous discussion!) My question is what file do I upload to the LMS to get those results. I tried the SL.html but all that does is give me a black screen. See below. I feel like I'm missing something but can't figure it out. I've also included the Test SL file. Thanks for your help.
