Hi,
I posted earlier, but have been troubleshooting it since. I feel like I made a lot of progress with the options inside my scroll panel. But just came to a new issue where the options that are supposed to be hidden, keep reappearing. This is happening to the two questions "Are you experiencing radiating pain?" and "Who do you think was responsible for your fall?"

I'd love to take a look, but the file didn't make it. You have to use a browser and not a phone to get attachments to work.

I did it from my desktop earlier, I'm trying again! Hopefully this will work!
I took a slightly different approach. Please see if it resolves your issue. I renamed some objects to make it easier for myself. I hope that's okay with you.
