Unable to Push Score to LMS

It sounds like your setup is correct, but be sure to double-check your Tracking and Reporting features to confirm the correct results slide is selected and all the right questions are tracked.
You can also recreate the results slide to reset the variables just in case something changed while editing. If you get stuck, feel free to share your file with our support team to help you troubleshoot the issue!
