Hello Fatma - This is not a built-in feature of the software. Perhaps the ideas shared in this thread will assist you in creating something that will work for you.
I just knew when I came across an activity with multiple fill-in-the-blank fields to complete on a side we had a problem. This should be a staple functionality in the software by now, right? A look back on the forum would certainly indicate the need. The ideas proposed 7 years ago in the linked thread aren't a viable work around for several reasons. I respecfully ask, how could a seemingly "must have" feature gone under the radar for 8 years now?

The entire community must be dilignent. Leslie stated 2 years ago..."I'm not seeing many feature requests for this functionality, so it's not bubbled up to the top of any radar."
I submitted this request over the years. The work-around is clumsy and takes time to work correctly. Having multiple fill-in-the-blank on one slide should be a standard feature.
In searching for the same answer Debra, I have to agree with you. Having the option to create a slide with multiple fill-in-the-blank choices (like a paragraph or a story) should be a standard feature. I am very surprised that this does not exist at this time.
We do have another feature request for the ability to create a slide with multiple fill-in-the-blank responses, so this is still on our radar. I have linked you to the feature request, and we will be sure to update you with any news should this make it onto our feature roadmap.

I also need that feature.
