Print Certificate (JavaScript) and mandatory fields

Hello everybody,

first of all, I have no knowledges about JavaScript and I have the following situation:

I've created a quiz and a certificate-slide where the users can write their names and dates in. This certificate-slide has a print-button and should be printable.

1. How can I make some text entries mandatory and only printable when they are filled out by the users?

2. How can I make the certificate-slide printable?

I've tried to paste the "printcertificate.html" from

http://tracycarroll.net/free-print-certificate-template/

into my published folder but I don't know where I have to paste this in? In the normal folder it doesn't work.

I hope anyone can help me :-)
