Ungraded Survey Help

Hello,

I'm working on creating an eLearning course question that isn't graded but pulls the answers users select into a summary page. The design ask is to enable the users to select a single answer in the first question, and up to 3 answers for the second question. 

On the summary slide, I need to display the answers users selected (up to 4) and also provide a spot for them to enter their final thoughts. 

Can someone please help me figure out the best way to do this? Please see attached.
