Course not triggering as completed

Depends in large part on your LMS. We had the same thing with ours (SuccessFactors, from SAP) and while it happened randomly, it happened more with courses published to SCORM 2004. We republished these to SCORM 1.2 and then the issue mostly disappeared.
Our LMS team has requested that whenever possible, we do not use the "Exit Course" button in the player if we are publishing to SCORM 2004. Our group uses that button a great deal so we publish to SCORM v1.2, and we have not had the problem occur in over a year that I can recall.
