Exporting Lesson Information to a Smartsheet

I am looking for information on how to export course information to a smartsheet using an embedded smartsheet form. The form would appear in a layer on several different slides so the learner can send feedback about issues with the content. We already have the form sending input such as comments and data from radio buttons but wonder how I can access other items from within the course in general. 

Examples I would like to include are the Course Name and Slide Title. 

Any ideas of how to capture those specific items?
