Graded Survey

Question

I'm trying to make a graded survey for a Mental Health E-Learning.&nbsp;
Is there a simple way that this can be done in storyline? i have included a screenshot for reference. The course wont be feeding back to the LMS its just more for the Learners guidance and advice.&nbsp;
If they score say 0-10 or between 11-14 and so on i want them to receive specific response/advice. I did think about using a multiple choice question template, but there's 5 toggles that are required on screen.&nbsp;
