Hey bylittlelearnin! I wonder if I could help you dig into this as I have resolved a lot of these issues that randomly occur in the past with us. Want to shoot me and email and see if we can track it down? Typically it happens when the Review item that was published is being overwritten and the state of that course gets stuck when processing.
