I have build a module in Storyline 360 that reports to our LMS. It should report a multiple choice as well as short answer question. Some students are having reporting issues. On the LMS is showing zero time spent on the module, however, I can see their interactions. Consequently, the module reports failed. How could this happen? Is it LMS issue of Articulate reporting issue?

You mentioned some students are experiencing reporting issues. Does the course work as expected for some students but not others? If it's specific to certain students, they may want to check the system requirements.
Additionally, to determine if the issue is related to your LMS or file, you can troubleshoot in SCORM Cloud! If it works correctly there, then I suggest that you reach out to your LMS administrator.
