Looks like test 3 nearly did it for you! Would the per-attempt score history and completion only on pass be Moodle configurations? I've never used Moodle so not sure what the options are but that type of configuration is how its tracked in the LMS I use. I have a 'activity completion - passed only' option. For attempts management I have a 'force new attempt' option for when a previous attempt is completed which is how the attempt history is recorded. Does Moodle have something similar? Someone will most likely come in with a simple solution!
