Mailto client + Survey results + multiple Likert scales + Text-entry fields

Hey everyone!,

I've read up about other solutions to email survey results, and add a single Likert scale input. I could use some help though to determine how to add multiple Likert scales to have their results collected from each and then have those added to an automatically sent email upon a user clicking 'Send Results' (meaning no e-mail windows opening upon click).

Can anyone advise with this item?
