Unwanted Tool Bar in Storyline SCORM 2004 files

When our team publishes a Storyline file in SCORM 2004, an unwanted tool bar appears on the left (please see attached picture).

This tool bar does not appear with publishing in SCORM 1.2, and we have ensured the player appears as desired without the tool bar.

We have also tried publishing in different versions of SCORM 2004 to no avail.

Anyone have any idea how to remove it?
