gathering test-taker contact info

I am using Quizmaker '09 to create a timed certification exam which will be hosted by the SCORM Cloud LMS. I am new to both of these products/services and am having a good experience so far with both.
Here's my question: I want to gather some contact info from the test-taker before they start the timed portion of the exam. Is it possible to create some kind of registration page in QM that can be captured by the LMS? I could use survey questions and get one piece of info per slide, but I'm hoping there's a better way (and which happens before the clock starts ticking). Thanks in advance for your help.

Hi David and welcome to Heroes,
The only way to do this is to put the survey questions at the beginning of the quiz. Since the timer automatically begins on the first part of the quiz, you'll need to adjust the timer to allow for the additional time that it would take to fill in the information.
