The Hotspot question type isn't setup to allow the end-user to "zoom" into the image & select an item. If you'd like to see that implemented into a future release, please submit a feature request to us by using the link below:
Instead of using a hotspot question, what if you used a multiple choice question and then inserted images of each document from the form view > insert media (copy and paste for desired number of images). In this way, the viewer can zoom each image and select the correct option. See attached QM file.
