Need help in fixing a submit results that is not functioning

Hello,  I have been asked to fix a problem for a course that I didn't create.

To give you some context:  This is an online course for OHS and is mandatory for certain employees, so it is essential we have a mechanism for tracking who has passed the course.  We do not have an LMS at our work, so the designer who created this course,  was creative in how to keep track.  For the final exam - done in Articulate Quizmaker '09 -    participants need to achieve 75% in order to successfully pass the course.  The designer customized a submit results interaction, once a participant successfully passes the final exam, there is a "customized"  submit results tab, participants click on the submit results and what is supposed to happen (and happened in the past), an email is sent to the team who is tracking results, in which participants names & results are included.  The problem that has occurred, and I don't know when this started,  is the email is sent to the team but with no information in it, no name & no results.  The participant thinks everything is ok because they saw their mark (successfully passed)  "clicked" on submit results.  But there is no record of them having successfully done the course.  Help please !!  And thank you in advance.  Sandra
